About the role

As an Outreach Case Manager with the Older Persons Support and Connect Program, you’ll provide essential support to older individuals experiencing or at risk of homelessness. This part-time role (24 hours per week) is based in Broadmeadows and focuses on assertive outreach, short-term case management, and linking clients to vital services. You’ll also engage in community development activities to reduce isolation, build connections, and support clients to live independently.
