**ROLE OVERVIEW**:
**About the Role**

Seasons Living Sinnamon Park community is seeking competent, caring and passionate Personal Care Workers. You will be responsible for the delivery of high-quality personal care, domestic and social support to HCP clients. Seasons provides care and services 24 hours a day, 7 days per week, and as such have a variety of shifts available including mornings, afternoons, nights, and weekends.

**Core Responsibilities**
- Assist residents with daily living keeping with their care and services plan and policies and procedures, in a manner that encourages choice and independence.
- Ensure the privacy, confidentially and dignity of clients and residents.
- Identify and promptly report any changes in resident health conditions, care and service preferences and take appropriate action as required in keeping with PCW scope of practice and organisational policies and procedures.
- Maintain records and documentation to a high standard that is consistent with the scope of practice of a PCW, medico-legal requirements, comply with relevant program guidelines including DVA and HCP requirements and Seasons policies and procedures.
- Demonstrate Seasons’ core values in all interactions with clients and their families.
- Ensure medications are administered, documented, stored, and disposed of in accordance with regulations and organisational policies and procedures.
- Ensure medication errors are promptly reported as per organisational policy and process, and further advice from a senior clinician (RN or EN) with regard to next steps.
- Ensure ongoing compliance with Seasons policies and procedures.
- Ensure incidents, dangerous occurrences, near misses, accidents, injuries, or hazards are immediately reported, and appropriate response is taken to situations of risk or potential risk to health, safety and wellbeing of staff, residents, visitors, or other relevant persons.
- Participate in professional development and education opportunities to maintain professional currency and further develop care skills and understanding.
- Actively participate in developing cultural competence and demonstrate an understanding of individuals from a diverse range of cultures and backgrounds.

**Top Skills and Experience**
- Demonstrated experience in aged care, preferably in a residential setting (desirable).
- Strong commitment to promote choice and independence to residents and practice in a manner in keeping with the principles of person-centred care.
- Demonstrated experience, knowledge and understanding of HCP, AN-ACC funding process and requirements (developing).
- Demonstrated knowledge of the DVA Community Nursing Program, HCP, Aged Care Standards and Client Rights and Responsibilities.
- Outstanding clinical skills and ability to practice autonomously but within the scope.
- Knowledge and experience in Microsoft Office Packages with a high level of proficiency.
- Well-developed written, verbal communication and mathematical skills with the ability to manage and share information effectively.
- Demonstrated ability to build collaborative relationships with key stakeholders and take actions to meet resident’s needs and expectations.
- Demonstrated analytical and critical thinking skills.
- Exceptional time management and organisational skills with ability to prioritise and manage multiple tasks.

- Certificate III Individual Support or equivalent qualification (Aged Care/Community Care)
- Current First Aid Certificate and Cardiopulmonary resuscitation (CPR)
- Pre-employment screening (National Police History Check, NDIS).
- Adhere to mandatory vaccination requirements (Public Health Directions).
- Unrestricted Australian working rights.
- Open drivers’ license.
