We support clients to retain dignity, independence and positive wellbeing through the provision of individualised personal care and domestic assistance, leisure and social activities, clinical and nursing care and meal preparation and nutrition. We also assist clients to access allied health services, specialised equipment and home modifications. We place the client at the heart of everything we do and provide services with compassion and competency. ACI Home Care operates throughout South West and Western Sydney. Facilities include a respite house, day care centre and activity hub, and accessible minibus. **THE ROLE** ACI Home Care has recently received approved provider status and is looking for an experienced Home Care Manager (Clinical Lead) who has the energy, experience and leadership capabilities to drive the strategic and operational direction of the business, whilst coaching and mentoring the team through a projected period of growth. The Home Care Manager (Clinical Lead) accountabilities include: - Lead and grow a financially viable service with a focus on quality care for our clients. - Provide oversight of home care services, including the delivery of nursing and personal care, community and social support, assistance with daily living, and other services in line with Schedule 3 of the Quality Care Principles. - Ensure the safe and effective delivery of services that are compliant with the Home Care Package program operational guidelines, aged care legislation and clinical standards. - Provide leadership, coaching and guidance to a multidisciplinary team of community based nurses, care coordinators, rostering officers and agency staff, and embed clinical excellence and continuous improvement within the team. Nurture a positive and accountable team culture. - Monitor and manage client experience and satisfaction. - Lead regular staff meetings and Chair the monthly Senior Management Team meeting. - Track and report key program and financial data to the Managing Director. **ABOUT YOU** - Proven team leadership skills and a sound understanding of clinical governance, Home Care Packages, Aged Care Quality Standards and consumer rights. - Experience in the assessment, planning and management of care for older clients or patients, including contemporary clinical nursing skills such as geriatric care, catheterization, continence management, skin integrity and wound management, pain management, delirium management, dementia, palliative care and other skills relevant to the care of older people. - Financial literacy with experience in budget management and financial reporting - Demonstrated ability to meet KPIs and financial targets - IT skills and demonstrated experience in client or patient management software - Ability to work autonomously with mínimal supervision - Willingness to be flexible according to the needs of the clients and service.
