**Role Overview**:
This is an exciting opportunity to join UnitingSA’s team as an NDIS Support Worker. This position involves assisting people of all ages with a wide range of both day-to-day, and fun activities such as shopping, attending appointments, going to the football, going to the cinema, going fishing, and much more. This position will involve travelling to and supporting participants in and around the Adelaide suburbs.

The NDIS Support Worker will assist participants in their own home and out in the community to explore opportunities and participate in activities that will enable them to increase confidence, skills and independence, and empower them to achieve their goals.

Due to expanding the services there are multiple permanent positions available, offering a range of hours to successful applicants, from between 15-76 hours per fortnight. This position may also require being available to work on weekends.

**Key Responsibilities**
- Develops a practical understanding of:

- The range of goals and aspirations that might be identified by an individual;
- The process of enabling supported decision making; and the customisation of service offerings/supports and procedures
- An understanding of the person-centred approach
- Assist participants to address their practical straightforward needs and expectations
- Provision of community support and/or training
- Communication and liaison with participants, family and carers
- Working with participants with complex and multiple issues
- Working directly with stakeholders to support client outcomes
- Maintain evidence-based records in relation to the participant contact and professional documentation.
- Identifying and accessing community resources to support participants.
- Use relevant communications and technology systems, including for creating/maintaining service records for billing purposes.

**Skills & Experience**:

- Minimum Cert III Individual Support
- Minimum of 2 year professional experience (Desirable)
- Cert IV in Mental Health or peer support (Desirable).
- Knowledge of issues facing people with disability particularly mental health.
- Knowledge of all relevant legislation, standards and NDIS pathways.
- Well-developed communication and interpersonal skills including negotiation and support decision making.
- Knowledge of case management, and working in groups.
- Excellent organisational and planning skills.
