**About the role**

This role joins an expanding Intrinsic At Home team to deliver a high standard of Occupational Therapy service to all clients in the Community.

This is an exciting role with a broad caseload. Whilst you’ll have the autonomy to structure your own day to meet key deliverables, you will also be supported by our experienced management team. Together you will deliver patient-centred, evidence-based care in an environment that promotes continuous quality improvement.

This role will include:

- Managing a wide variety of client presentations across many funding areas including HCP, CHSP, and Residential Aged Care.
- Clinical assessments and treatment of physical and neurological conditions
- Providing simple and complex equipment prescription
- Environmental assessments and home modifications to minimise the risk of falls and improve access
- Liaising with healthcare professionals, such as GPs, nurses and occupational therapists to ensure care plans are formulated and monitored
- Participate as a member of the multidisciplinary team in assessing clients level of function and care needs, setting care goals, and providing appropriate treatment and interventions

**Qualifications & experience**
- Qualification in Physiotherapy with current AHPRA registration
- Previous experience working as an PT in a community setting, home modifications and equipment prescription, desirable but not essential
- Ability to work autonomously as well as within a multi-disciplinary team
- Excellent interpersonal and communication skills (written and verbal)
- Current drivers’ licence and access to vehicle
- Current First Aid & CPR Certificates (or willingness to obtain)
