What you'll be doing

To provide Allied Health clinical expertise, leadership and support the coordination, access, and continuous improvement of community-based allied health services delivered through Integrated Virtual Care Services, including but not limited to the Out of Hospital Care (OHC) Programmes – ComPacks, SASH, and EoL, MNC Virtual Care, Aged Care Outreach Service and Central Intake.

This role strengthens care delivery through integrated multidisciplinary collaboration, ensuring timely and person-centred responses for clients with chronic conditions, rehabilitation needs, and complex care presentations. It supports both virtual and face-to-face care pathways, contributes allied health expertise to urgent and unscheduled care responses, and drives early intervention and hospital avoidance across a range of settings.

This position will be located within Mid North Coast LHD, the successful applicant will be required to work onsite at either our Coffs Harbour or Port Macquarie Hub.

About you

  • Qualifications eligible for membership of the Australian Association of Social Workers; or qualifications recognised for registration with the Occupational Therapy Board of Australia; or qualifications recognised for registration with the Physiotherapy Board of Australia.
  • Demonstrated advanced clinical reasoning skills with knowledge and experience working with clients across the care continuum, including chronic disease management, post-acute care, rehabilitation, and end-of-life care
