**About the role**

An opportunity exists for a Registered Nurse or a Social Worker in the role of Voluntary Assisted Dying Regional Care Navigator. This role encompasses the whole of the Grampians region.

Victorians nearing their end of life who meet defined eligibility criteria are able to request assessment for Voluntary Dying under the Voluntary Assisted Dying (VAD) Act 2017.

The VAD Care Navigators are a small team of highly skilled clinicians, spread across Victoria. They are the point of contact for members of the public, health practitioners and health services across Victoria seeking information and support in accessing or delivering VAD.

**Key Responsibilities**
- Provide practical support to people, carers and families in understanding the legislation and implications in a persons specific context.
- Explore a persons specific needs and goals of care and assist with identifying appropriate service referral pathways.
- Support VAD assessing medical practitioners in working through legislated processes.
- Help build skills and capacity in the broader health service by providing ongoing professional education, upskilling, working with health services in determining their level of participation and reviewing or implementing appropriate policies and procedures.

The GH VAD care navigator is a sole position with the Grampians Region, working closely and with the support of the statewide service and Service Manager based in Melbourne.

The successful applicant will spend some time in Melbourne with other members of the team learning the role.

**Skills and experience**
- Registered Nurse with AHPRA or eligibility to be a member of AASW
- Prove ability to build relationships
- Knowledge of the legal requirements as per the relevant Act
