We are looking for a full-time Disabilities Services Officer to join our team in NSW.

**Responsibilities & Duties**

The duties for this position include:

- Ensuring that clients/participants are provided with and can access high-quality support, education, community access, skill development training and social support.
- Collaborating as part of a team to optimise clients' well-being through health care plans.
- Building strong relationships and ensuring client best interests are met.
- Working closely and collaboratively with clients, their families, allied health professionals and care team to ascertain client requirements, provide feedback and accommodate changes to client mobility, needs and plans.
- Providing access to innovative community participation programs and ensuring client engagement and access to community.
- Ensuring clients are assisted through life change transitions depending on individual ability and needs (e.g. school leavers moving on to the workforce)
- Engaging in assessment (as well as re-assessment) and liaising with families, allied health professionals and care team to ascertain needs and plan, develop and implement programs.
- Developing support plans to assist clients in achieving goals relating to education and community participation/access.
- Monitoring/reviewing delivery of services as per care plans, interviewing clients, ongoing assessment of suitability, assessing difficulties and reporting on progress and issues.
- Ensuring clients engage in appropriate education programs according to individual needs (e.g. road/traffic safety programs, public transport access programs, money handling programs, crisis management programs etc.).
- Engaging/referring to additional service providers as required (e.g. behavioural support specialists to assist in behaviour management).
- Liaising with other professionals to identify issues and develop/implement solutions.
- Periodic review of support plans together with families, allied health professionals and care team; review progress, identify issues and update plans as required.
- Maintain accurate and compliant records.

**Salary**

The salary for this position is $75,000 per annum plus superannuation.

**Requirements for the Role**

To be considered for this position, you should have:

- At least a relevant diploma or higher qualification
- 1-2 years of relevant work experience in the disability industry
- Current unrestricted driver's license.
- Current First Aid Certificate with CPR (desirable).
- Effective written and verbal communication skills for clear case notes.
- Strong interpersonal skills for team collaboration and autonomy.
- National Police Check (no older than 6 months).
- NDIS Worker Screening Clearance.
- Covid Certificate
- Working with Children Check (WWCC)
