**About the role**

We currently have a permanent part-time opportunity for a skilled, enthusiastic and dedicated **Commonwealth Home Support / DVA / VHC Care Case Manager** to join our Home Care Team in Launceston willing to travel to Shearwater on a regular basis.

As a **Commonwealth Home Support / DVA / VHC Care Case Manager**, you will have an opportunity to:

- Provide day to day management of a client case load, care planning and provision of consumer directed services to clients
- Make a real difference to our consumers lives
- Enable our consumers to remain in our own homes

**What you bring to the role**
- Relevant experience in case management, community services, aged care or equivalent **(Essential)**:

- Knowledge of CHSP and VHC funding **(Essential)**:

- Experience in service delivery to clients living in the community
- High level planning and organisational skills
- Well-developed communication skills
- A desire to create a positive experience for our consumers
- A current unrestricted drivers license **(Essential)**
