
Support Coordinator

**Position Overview**
The Support Coordinator works with NDIS participants to help them understand and implement their NDIS plans, connect with appropriate supports, and build their capacity to live as independently as possible. This role involves working closely with participants, their families, and service providers to ensure participants are achieving their goals and receiving quality support.
**Key Responsibilities**
**Plan Implementation**
- Assist participants to understand the details of their NDIS plan and how to utilise funding effectively.
- Identify and connect participants with suitable service providers and community supports.
- Coordinate a range of supports to ensure a holistic approach to the participant’s needs.
**Capacity Building**
- Support participants to develop skills and confidence to manage their supports independently where possible.
- Promote participant choice, control, and independence in decision-making.
**Monitoring and Review**
- Regularly monitor the effectiveness of services and supports in achieving participant goals.
- Assist with plan reviews, including gathering reports, evidence, and feedback from providers.
**Advocacy & Problem-Solving**
- Act as an advocate for participants when liaising with service providers, government agencies, and community services.
- Resolve issues or barriers impacting the delivery of services in a timely and participant-focused manner.
**Administration & Compliance**
- Maintain accurate case notes, records, and reports in accordance with organisational and NDIS requirements.
- Uphold confidentiality, privacy, and safeguarding standards at all times.
**Qualifications & Skills**
**Essential**:
- Excellent communication and interpersonal skills.
- Strong organisational and problem-solving abilities.
- Ability to work independently and manage a diverse caseload.
**Desirable**:
- Knowledge of local service providers and community networks.
- Demonstrated experience in disability, community services, or a related sector.
- Strong understanding of the NDIS, its principles, and processes.

**Salary & Benefits**

Pay: $40.00 - $45.00 per hour
Expected hours: 15 - 38 per week
