We are currently looking for a permanent part time Lifestyle Assistant to join our Southern Cross Care QLD family, based at our large and modern Duhig Village Residential Aged Care Home in Holland Park.

In this rewarding role, in conjunction with other members of the care team, you will assist with the implementation of Leisure and Health programs for our residents.

**What are the responsibilities?**
- Actively engage with residents to design and implement individual Diversional Therapy Plans incorporating various Leisure and Lifestyle programs to support, challenge and enhance psychological, social, emotional, spiritual, cognitive and physical wellbeing
- Liaise with Registered Nurses and/or other relevant staff to understand the varying clinical needs and restrictions some residents may face
- Observe customer responses to programs, report notable finding to Clinical and/or other relevant staff and ensure necessary changes are made
- Continually evaluate customer plans and incorporate new approaches and strategies as appropriate
- Ensure compliance with accreditation outcome in relation to the Leisure and Lifestyle program
- Manage the activities budget and coordinate appropriate resources and equipment as necessary
- Assist with customer personal care needs to the extent necessary to ensure safety, comfort and the satisfactory completion of planned programs
- Actively participate in Diversional Therapy related policy and procedure formulation and improvement

**What you will bring to the role?**
- You will hold a Certificate IV in Leisure & Health or equivalent (desirable)
- You will have an understanding of Aged Care and Residents rights and responsibilities
- You are dedicated to caring for older people to live their best lives
- You will work well within a team environment and have a friendly approachable manner
- Strong attention to detail and outstanding communication skills
