**About the Role**
Our HCP Care Partners, are integral to delivering tailored & trusted support to our clients, helping them to achieve their personal goals and getting to know them to ensure their health & wellbeing needs are taken care of. They are the client’s advocate within our dedicated team.
You will support with passion and commitment up to 50-60 clients, continuously assessing their health & wellbeing and come up with person-centred solutions in consultation with them and a diverse range of stakeholders to ensure high-quality care.
Beyond supporting our existing clients, HCP Care Partners are very important to meet prospective new clients, giving them a sense of relief that help is in sight and to build trust that they can count on us to focus on great solutions in partnership with them.

**What You’ll Be Doing**
- Crafting personalized care plans tailoring your approach to each client’s individual needs, goals, and preferences.
- Building strong client relationships by developing deep, meaningful connections with them and understanding their unique stories and what drives them.
- Growing your client portfolio and managing your own client base demonstrating why we’re the best choice for homecare
- Driving community impact by actively contributing to our reputation by delivering exceptional, person-centred care.
The role is based out of our Tullamarine office with WFH flexibility and travel to visit clients as required.
**About You**

Here’s what makes you a great fit:
- 3+ year of relevant experience in a similar role, helping you to hit the ground running.
- Knowledge, understanding and experience in Aged care, Home or Community care
- Passion for outstanding service with a genuine desire to go above and beyond in every interaction.
- Strong communication skills with the ability to connect deeply with your clients and build lasting relationships.
- An innovative mindset with the ability to think creatively and contribute ideas that set us apart.
