**Calling All Support Coordinators** ELS have been supporting its participants for over five years, we are presently experiencing growth in participant numbers therefore we are on the lookout for a committed, professional, and compassionate individual to join our small team with the opportunity to proceed into management and supervisory roles. We are currently looking to hire someone who has experience as a Support Coordinator working supporting people with physical, intellectual, and psychosocial disabilities ensuring that they are implementing their NDIS plans, building their capacity, and achieving their goals. **Location - Western Sydney.**

Our mission is to employ like minded individuals who will uphold our companys morals and values, understand individual need and ensure high quality support is delivered. Your role as an ELS Support Co-ordinator will be as a valued team member to help provide services to support clients’ individual needs both in their homes and in the community and to provide back up client support if a care worker is unavailable
- 2 days p.w (for now) can be worked from home or office. Can be fitted into school hours. Work/Life balance. Laptop and Phone provided
A manageable workload
Have a current driver's license and own car
Have 6 mths - 1 year minimum experience working as a Support Coordinator
Fully vaccinated.

**Responsibilities will Include**:Build rapport and consult effectively with staff/management, participants, their family, and friends to achieve a positive outcome and ensure a smooth service delivery flow throughout the participants' plan.
Assist with liaising with participants' support networks including allied health, families, and friends to explore new goals, ambitions, promote and empower participants to achieve their goals and provide ongoing, timely communication.
Flexible and adaptable
Understanding mental health and how it can impact individuals
High attention to detail
Excellent skills in writing/reading observations and assessments
Strong ability to multitask and build rapport with individuals from different background
**Essential Requirements**:A Qualification in Community Services or Disability Services or related industry. Mental Health knowledge would be helpful
6 -12 months minimum experience in providing support coordination services, or similar role, support people with physical, intellectual, or psychosocial disabilities
COVID Vaccinations
A safe, reliable vehicle with comprehensive car insurance
WWCC or willingness to obtain
Police Check or willingness to obtain
NDIS Worker Orientation or willingness to obtain
The role will involve working with participants with both low, moderate, severe needs.

**Job Type**: Casual

**Salary**: $45,000.00 - $60,000.00 per year
