About the role
Your Community Health is seeking an enthusiastic individual to contribute to our Social Support and Men's Shed Programs. Assist in the day-to-day implementation of the programs under direction of the Program's Coordinator and Manager. Support carers by providing respite, assistance, advice and understanding on issues relation to the caring role.

**Duties**:

- Partner with consumers to encourage consumer directed decision making to plan, implement and evaluate group activities and gentle exercises.
- Work collaboratively with the Social Support Worker team to provide activities that enhance participant's wellbeing, encourages independence and reablement and meets their diverse needs.
- Value and embrace the diverse needs of individuals and ensure the program reflects diversity and inclusive practices.
- Support quality consumer documentation requirements.

Skills & experience
- Enjoyment working with older people and people with disabilities living in the community, including multiple sclerosis and dementia.
- Understanding and appreciation of the needs of participants who are aged and/or have a disability.
- Sensitivity to issues relating to ageing, physical and intellectual disabilities, mental illness and diverse cultural needs.
- Commitment to safety and quality assurance for participants and staff.
- Verbal and written communications skills when working with people in community setting.
