**Salary Classification**: Social Worker Level 1-2.
**Remuneration**: $35.78-$47.28 per hour + superannuation + annual leave loading + salary packaging options.
**THE ROLE**:
Calvary Mater Newcastle is currently seeking a Social Worker Level 1-2 on a part time fixed term contract. This position is a fixed term contract until November 2025.
The successful applicant is responsible for the provision of Social Work services and the maintenance of professional social work standards. This responsibility shall include provision of a social work service to patients / clients and their families, liaison with other health care professionals within the Hospital and with other external agencies, and contribution to the development of policies, procedures and services in the Social Work Department.
**DUTIES**:
Provision of professional Social Work services as part of the Calvary Mater Newcastle Social Work Department. While principally providing service to clinical areas as determined by the Head of Social Work, provide support to Social Work team by assisting in providing services to other clinical areas as required during times of staff leave.
Provide comprehensive psychosocial assessments of patients attending the hospital.
Provide support, counselling and other evidence based intervention strategies to enhance social functioning. These may include individual and joint interviews, case conferences, family meetings, home visits, group sessions, practical assistance, and referral to community agencies and follow up of patients/family/carers after discharge.
Maintaining a high standard of written communication, with accurate and timely medical/clinical record entries and documentation as required, in accordance with Calvary Mater Newcastle policy.
Facilitate discharge planning in consultation with multidisciplinary team.
**ABOUT YOU**:
Bachelor of Social Work or other equivalent qualification and eligible for membership of the AASW
Demonstrated effective interpersonal and verbal and written communication skills in building and maintaining professional relationships
Demonstrated ability to work effectively as a member of a multidisciplinary team
Demonstrated ability to liaise with and maintain effective links with relevant external organisations
Demonstrated ability to effectively prioritise and organise own work / caseload
Demonstrated commitment to evidence based practice and ongoing personal and professional development
