**About the role**

We are seeking a Family Connect and Support Worker who will be responsible for delivering a high standard of service to vulnerable children, young people and their families by providing comprehensive assessment, engagement, short term case planning and case coordination as required with the overarching goal of identifying and referring families to relevant supports within a time frame of 16 weeks.

This role exists within our Child, Youth & Family Services and will work within a large and supportive team environment.

This position will require you to work across the Northern Sydney region.

**Duties**
- Promote a 'risk-aware' culture where staff proactively identify, report and address all forms of risk to clients, staff and CatholicCare.
- Conduct thorough assessments of families and young people to ensure the child’s and family’s needs are addressed.
- Work with families with children on a range of issues related to child protection such as Domestic Violence, Mental Health and Alcohol and drug misuse in the family context and child safety.
- Provide outreach to engage with families and young people at an early stage

**Skills & experience**
- Demonstrated experience and skills in information provision, intake, assessment and referral of working with families in need of support services
- Excellent understanding of child protection issues and client vulnerabilities
- Able to engage with parents and young people in order to develop case plans and goals
- Able to work as part of a team
- National Criminal History Check
- Working with Children’s Check
- Current NSW Driver’s Licence
