**Description**

This is an opportunity to join the Attained Care team which involves support to assist NDIS participants with their Disability. These shifts are in **Sutherland District and St George/Bayside Council.**

You will be supporting NDIS participants with empathy, humanity, and care, with the goal of the Disability Support Worker to support the participant with their journey working towards leading a self-sufficient and fulfilling life.

Participants have various support needs, and depending on the participant, your role as a mental health support worker/ disability support worker will involve supporting a participant to increase skills and personal capacity, exercise choice and control including motivation, strengths, resilience, and decision making.

**What are the duties and responsibilities of a Disability Support Worker**

Providing care and support to clients in line with their personalised care plans, including:

- Personal tasks and assisting with self -care.
- Domestic assistance
- Respite
- Social support / community access
- Transport
- Meal preparation
- Group participation
- Life skills development
- Coaching to increase recovery skills and build capacity, including motivation, strengths, emotional resilience, and decision making.
- Strengthen and enhance participants capacity to coordinate supports,
- Assist participants to decide on what actions to take to achieve goals
- Completing Progress Reports as per NDIS requirements
- Writing comprehensive case notes and do this articulately, clearly, and concisely

**What are the required skills and qualifications?**
- Cert III Individual Support, Mental Health, Aged Care, Community Services, Disability Work or similar.
- Minimum 6 months relevant experience
- Experience supporting People with Disability.
- Demonstrated Continuing Professional Development.
- Knowledge of continuous quality improvement principles
- Minimum Australian Probationary Driver’s License, fully comprehensive insurance.
- Car Registration
- Current Working with Children's Check ,police check or NDIS workers clearance check
- Current valid First Aid and CPR Certificate or willing to apply.
- Current NDIS Worker Screening Check or willing to apply.
- COVID-19 Training and Control Certificate or willing to obtain.
- NDIS Orientation Certificate or willing to obtain.
