**About Social Work Services at Monash Health**

The Social Work team across Monash Health delivers a broad range of Social Work services for clients across the lifespan in the inpatient and outpatient setting.

We now have three full time permanant opportunities for experienced Grade 2 Social Workers to join the team. Two positions are based at Monash Medical Centre, Clayton and one at Moorabbin Hospital and you may be required to work at other Monash Health sites from time to time.

**About the roles**

In any of these roles you will be working with medical oncology, surgical and general medicine patients, from a diverse adult age group. You will support patients and their families around adjustment and crisis intervention and will also address complex social issues impacting on their recovery. This may include providing counselling.

These clinical roles offer an opportunity to showcase your skills in working with people from a range of diverse backgrounds as well as completing complex psycho-social assessments and developing patient centred care plans.
- Deliver and drive Monash Health strategic intent to be relentless in our pursuit of excellence across Social Work and Allied Health.
- Work closely with the Social Work Manager to provide streamlined services across all campuses and lead designated services/programs/portfolios.

It is a requirement of this role to participate in the on-call rosters.

**About You**

You are a dynamic, innovative and flexible clinical practitioner with a good understanding of the treatment and psycho-social issues facing individuals with chronic illness and their families.

You have an understanding of the aged care sector, chronic health conditions and related services and referral pathways. With demonstrated skills in psycho-social assessments, problem solving, communication and hospital discharge planning, you ensure that high quality Social Work services that place the client at the centre of care are provided.

Education, quality improvement and evidence based practices are your key drivers. Your proven clinical experience enables you to bring knowledge of healthcare reforms and initiatives as well as an appreciation of the challenges facing public healthcare now and in the future.

Previous hospital social work and oncology experience is highly desirable.

**What you need**:

- You must be eligible for AASW membership including continuing professional education
- Proven experience in providing social work in acute and subacute settings
- Knowledge and experience working closely with inter-professional teams

As travel acriss Monash Health sites will be required, you will need a current Driver's Licence.
