**About Chorus**
Chorus is a WA-based organisation with close to 1000 staff and volunteers who support seniors, people on the mental health recovery journey, and those living with a disability. Our purpose is to enable people to live the life they choose at home and in the community. We have designed a structure that focuses less on management and hierarchy and more on the things that matter - relationships, customers, and communities Our network of small local teams enables Chorus people to be more connected to each other and feel a sense of belonging.
**About the role**
We are currently looking for a **Customer Advocate (Customer Coordinator) **to join our team. The purpose of this role is to support seniors and NDIS customers to remain in their homes. This role is based in the central Perth area and provides flexibility to work from home, in the office, and out supporting customers in the Stirling area. At Chorus all team members have a role to play in customer-facing time. This ensures that we keep the customer at the centre of everything we do.
**No day is the same and you’ll get to enjoy a variety of work which can include**:
- Working in partnership with customers and their families to design/review their support plans in line with their goals and approved a funding package
- Onboarding new customers for your local area
- Developing and nurturing relationships with current and potential customers, their families, and other key stakeholders (e.g. NDIS planners, LACs, and government agencies) in relation to service delivery and outcomes
- Building your team’s understanding of customer goals, to underpin the delivery of an exceptional customer experience
- Helping customers understand what Chorus can offer and establishing clear service expectations
- Coordinating your local area’s delivery of services to customers
- Managing your workload effectively to ensure customers receive timely and responsive service from your Local
- Managing client budgets
**This is what you will need**:
- Experience in coordinating services and supporting people living with disability, on the mental health recovery journey, and/or seniors
- Experience in supporting customers with complex mental health needs
- Understanding of NDIS and aged care funding models, plan interpretation, and implementation
- Previous Experience as a support worker is desirable but not essential
- An NDIS Worker Screening Check
- Current Western Australian Drivers Licence
- Your own vehicle
- Rights to work in Australia
- A qualification in this area is desirable but not essential
- Ability to work both autonomously, as well as within a high-functioning team
- An awesome work ethic, which includes attention to detail and financial acumen
- Strong written and verbal communication skills
- Team player and collaborative problem solver
- Experienced in the Microsoft Office package
