We are seeking a compassionate and organized Support Coordinator to join our team. In this role, you will be responsible for assisting clients in navigating the NDIS, helping them achieve their goals and improve their quality of life. You will work closely with clients, their families, and service providers to ensure that clients receive tailored support that meets their unique needs.

Key Responsibilities & Duties:

- Knowledge of NDIS Legislative framework and Support Coordination processes
- Capable of working individually.
- Conduct initial assessments to identify client needs, goals, and support requirements within the NDIS framework.
- Develop and implement individualised support plans in collaboration with clients and their families.
- Facilitate access to appropriate services, supports, and funding under the NDIS.
- Provide ongoing support and coordination for clients, ensuring they receive timely and effective services.
- Advocate on behalf of clients to ensure their rights and preferences are respected
- Monitor and review client progress regularly, adjusting support plans as necessary.
- Assist clients in connecting with community resources, social activities, and other support networks.
- Maintain accurate and up-to-date client records and documentation as required by NDIS compliance.
- Collaborate with interdisciplinary teams and external providers to ensure a holistic approach to client care.
- Participate in training and professional development opportunities to enhance skills and knowledge related to the NDIS and client support.
- Collaborate with interdisciplinary teams like psychologists, physiotherapists, occupational therapists, and medical clinics to provide comprehensive care.

About you:

- A minimum of one year's experience providing community services and working as a support coordinator or in a similar role.
- Valid drivers license, First Aid certificate, Police Check, Blue card, and NDIS Worker screening check.
- Relevant qualifications in Social work, Disability, community services, or a related field.
- Experience in support coordination, case management, or a similar role within the disability sector is highly desirable.
- Able to handle complex situations and resolving issues.
- Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
- Strong organisational and time management skills, with the ability to manage multiple clients and tasks effectively.
- Empathetic and person-centred approach to supporting clients from diverse backgrounds.
- Able to work individually.
- Friendly & Approachable.
