**About the Role**

**What You’ll Do**:

- You will help Community Connections Program participants overcome challenges and increase independence.
- Focus on social inclusion activities and building social and support networks.
- Aim to utilize knowledge of the local community and case coordination skills to achieve program outcomes.
- Assist participants in identifying barriers to forming social connections.
- Provide support for engaging in inclusion and capacity-building activities.

**What You’ll Need**
- Tertiary qualification in a Human Services related field desirable.
- A current and unrestricted SA driver’s license & registered and road worthy vehicle.
- Excellent interpersonal skills to negotiate and liaise effectively with staff, clients, volunteers, internal and eternal partners and service providers.
- Ability to provide client centred, holistic and strength-based case coordination support.
- Excellent organisational skilled and demonstrated capacity to work flexibly and within tight timelines.
