Under s25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal and Torres Strait Islander community.
About the Role
Work as part of the Child Health Service multi-disciplinary team and relevant key stake holders to advance the health and strengthen the capacity of Aboriginal and Torres Strait Islander families across Child and Youth Community Health Service (CYCHS).
With the appropriate Child Health Service Cluster Nurse Unit Manager (NUM)/ Clinical Nurse Consultant (CNC), develop and facilitate pathways for Aboriginal and Torres Strait Islander families to access and use the Child Health Service.
Participate in service quality improvement and research activities, attending service requirements that support and promote evidence-based practice and critical thinking. This will include but is not limited to Care Coordination, team meetings and cultural support within scope of practice.
Work collaboratively with the Child Health Service Cluster CNC/NUM to maintain a clinical case load to the full scope of Health Worker practice. This will include but is not limited to:
Engagement with Aboriginal and Torres Strait Islander clients during the perinatal period to assist their access to the Child Health Service
Work collaboratively with the other Aboriginal and Torres Strait Islander Advanced Health Workers within the Child Health Service.
Facilitating client referrals to appropriate services in both government and non-government sector as required
Home Visiting clients
Presentation and contribution at team Care Coordination meetings
Assisting families to self-weigh their infants
Cultivating community engagement and participation in relevant Aboriginal and Torres Strait Islander cultural activities with respect to local elders, leaders, and communities.
Set goals for personal and professional development and participate collaboratively in the Performance Coaching and Development process (PC&D) as per CHQ Policy
Proven experience in leading, managing and mentoring a small to medium sized team. (NHS: Leading change through people).
Proven analytical, conceptual and problem-solving skills and a proven ability to work in a team environment developing effective working relationships with team members.
