**About the role**

The lifestyle team are responsible for implementing activity programs that positively impact the lives of our residents, in both a group and one on one setting. Under the guidance of the Lifestyle Coordinator, you will carry out lifestyle assessments that are relevant to each residents needs. An engaging role where you make a positive impact to our residents wellbeing everyday.

This role would be working in our Memory Support Unit, supporting people living with dementia.

**The Caboolture Home**:

- Large 171 bed home located in a quiet residential precinct next to the Caboolture Public Hospital
- Close to shops and public transport
- Staff parking at the facility
- Fantastic leaders in the home that support and encourage you to grow your career

**A day in the life of a Lifestyle Assistant may see you**:

- Assist in carrying out lifestyle assessments of residents
- Actively participate in the delivery of activity programs
- Ensure complete and accurate documentation reflecting any changes to resident’s status
- Liaise with residents and their families

**What we look for**:

- Certificate IV in Leisure and Health (desirable)
- Experience in a similar role
- Positive and caring attitude
- Experience using a computerised Care Management System (desirable)
