**INTAKE CARE CORDINATOR -Mercy Palliative Care**
**ABOUT THE ROLE**:
- 2 years fixed term part time role
- 0.7 EFT
- Rostered Monday to Saturday 0800-1630 shifts
- Mercy Palliative Care, located at Sunshine office
- The role is responsible for the triage and coordination of referrals and scheduling assessment visits for Mercy Palliative Care’s community program.
- It may also include first visits to complex community patients
- The role requires a Registered Nurse with a comprehensive understanding of the underpinnings of palliative care and a commitment to the delivery of timely and appropriate holistic care
**ABOUT YOU**:
- 5 years’ experience in community palliative care
- Post graduate qualifications in palliative care
- Advanced clinical knowledge/ability to work as an integral part of a multidisciplinary team
- Excellent communication and problem solving skills
- Current Victorian Drivers Licence
