**About the role**

We are looking to hire a Disability Support Worker to assist the people we support in the Landsborough & Glasshouse regions. You will be a proactive and passionate support worker that is available to work across a 24/7 rotating roster, including sleepovers.

These roles require you to provide transport for clients (company provided vehicle), so a valid Australian Drivers Licence is essential. As a Disability Support Worker you will be able to make a lifelong positive impact to the lives of the people we support, helping them to maintain their independence and achieve their personal goals.

**Key Responsibilities**
- Day to day care (including personal care) and supervision
- Community access and transport
- Maintain and monitor Person Centred Plans
- Track and document an individual’s progress through thorough record-keeping
- Build rapport and collaborate effectively with the people we support and their families to establish positive relationships

**Essential Skills & Experience**
- Unrestricted Australian Drivers Licence
- Current First Aid and CPR Certificate
- Only those with prior experience in the industry supporting clients with challenging behaviours and/or relevant qualifications will be considered
- Experience within Aged Care is ideal
