**Social Worker (Acquired Disability) - Full-time Social Work role in our Toowoomba Office**

Rehability Australia is seeking an experienced **Social Worker** to join our **Disability & Case Management Team**, providing specialist disability and rehabilitation services to individuals living with **Acquired Disabilities - Brain Injury (ABI)** and **Spinal Cord Injury (SCI)**.

**About the Role**

As a **Social Worker in our Toowoomba team**, you will:

- Deliver **specialist social work and case management rehabilitation services** for clients with ABI and SCI.
- Conduct psychosocial assessments and develop **tailored support and rehabilitation plans**.
- Liaise with medical professionals, allied health providers, support coordinators, and community services to deliver high-quality outcomes.
- Advocate strongly for clients to access appropriate funding, services, and resources across schemes including NDIS, NIISQ, icare, TAC, DVA, and other insurers.
- Utilise **Flowlogic CRM** to manage client information and track service delivery effectively.
- Build and maintain **local disability sector connections** to improve service accessibility for clients.

**What We’re Looking For**
- Bachelor’s or Master’s degree in **Social Work**:

- **Active membership with the Australian Association of Social Workers (AASW)**.
- **Minimum 2-5 years’ direct experience** working with people who have sustained **Acquired Brain Injury or Spinal Cord Injury**.
- Proven skills in **social work**, **case management and rehabilitation planning**.
- Strong **knowledge of Toowoomba’s disability services sector** and established professional networks.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Experience using **Flowlogic CRM** software.
- A valid driver’s licence and reliable vehicle.
