**The Role**

This Case Manager role is allocated across all the programs in the Child, Youth and Families including; Home Based Care, Kinship, Intensive Case Management Services and Targeted Care Packages (TCP) in a generalist role. The case manager would maintain a case load of mix portfolio, dependent on the need across the program area.

**Key Responsibilities**
- Provide statutory case management services and care management to a range of children and young people residing both in out of home and parental care.
- Provide placement support, carer supervision and attending to all requirements to ensure carers are meeting accreditation requirements.
- Prepare reports as required for the Department of Families, Fairness and Housing.

**About You**
- Experience in outreach support and/or case management with Young People
- A tertiary qualification in Community Services, Education, Social Work or related field
- Quick and motivated learner
- Experience in contracted case management within DFFH, Child Protection or similar agency will be looked upon favourably
