**Role Overview**:
Join UnitingSA in delivering exceptional care Deliver tailored care promoting safe, independent living in line with Care Plans. Document provided support and promptly escalate any health concerns to ensure wellness and safety.

There are multiple opportunities available as Permanent Part-Time or Casual in the Community Aged Care.

**Key Responsibilities**:

- Deliver personalised, goal-oriented care for independent living at home and in the community.
- Escalate client-related issues promptly to a Coordinator or after-hours On Call.
- Communicate openly and sensitively with clients, families, and advocates.
- Actively participate in directed individual and group care activities.
- Promote UnitingSA positively both internally and externally.
- Manage time effectively and inform clients about scheduling constraints.
- Guide clients to their Coordinator for service changes or additional support.
- Provide tailored support to culturally diverse clients.
- Assist clients in communicating service changes to their Coordinator.
- Monitor and swiftly report changes in clients’ well-being to a Coordinator or after-hours On Call.

**Skills & Experience**:

- Experience working in a community home care setting with older people.
- Proven ability to effectively communicate with clients, their relatives, carers, and/or advocates, as well as substitute decision-makers.
- Experience in catering to the needs of older individuals from culturally and linguistically diverse backgrounds (CALD) - desirable.
- Demonstrated capability to work autonomously with mínimal direct supervision.
- Proficiency in comprehending and adhering to client Care Plans.
- Track record of establishing rapport with clients, their families/carers, key stakeholders, and fellow staff.
- Minimum qualification of Certificate III in Individual Support or Community Services - essential.
