Peer Community Support Worker - FT - North Ryde
**We have an exciting Full-time Peer Community Support Worker ( Mental Health) opportunity available in our HASI HUB, based in our North Ryde office.**

The Housing and Accommodation Support Initiative (HASI) is a state wide program that supports people with a severe mental illness to live and recover in the community in the way that they want to. Services delivered under HASI encompass a wide range of activities that build independence in daily life. Supports can include recovery planning, support to live independently, assistance to manage a tenancy, assistance to manage health and emotional wellbeing, as well as support to get involved in your local community.

**These positions involve shift work, and you will need to be available to work flexible shifts that may include a combination of mornings, evenings, and weekends.**

**Based in North Ryde office, you will report to the Team Leader in our Inclusion Team.**

**Key responsibilities will include**
- Collaborating with customers to develop individualised support plans that incorporate their identified wellbeing needs and goals
- Collaboration with **Local Health District** Community Mental health **Team** around **Customer clinical goals**:

- Complete comprehensive risk management plans **with** customers
- Providing support to customers to improve their independence by assisting with living skills, transport, social activities, medication and personal care, as well as other support as required **to achieve customer goals**:

- Supporting customers to explore their practical and complex needs
- Keeping customer records up to date daily using on line systems
- Developing and maintain professional relationships and networks with customers, families, caregivers and key stakeholders
- Seek out innovative ways to enhance the customer journey

**You'll also need**
- Technical literacy with proven ability to use Microsoft Office and systems for reporting
- Minimum Cert IV in Mental Health **Peer Work **or equivalent industry experience
- Availability to work across a rotating roster
- Valid NSW Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans
- Willingness to obtain an Australian Federal Police Check, WWCC and first aid certificate that meet New Horizons employment criteria prior to commencing with us
