Are you passionate about addressing health inequity and having a positive impact on young people in statutory care?

Western Health is seeking an Aboriginal Health Pathway worker to work within the Pathway to Good Health Program, as part of its Integrated Community Health Service. This is a 0.4FTE part-time, permanent position. Working within a dynamic and diverse multidisciplinary team and in collaboration with key stakeholders you will be supporting Aboriginal and Torres Strait Islander Children in Out of Home Care (OOHC) to access comprehensive health supports and address significant unidentified and unmet health needs.

You will be supported to progress your career with high quality supervision and professional development opportunities by working alongside medical and allied health clinicians from our specialist programs and clinics.

**To be Successful in this Role, you will have**:

- Knowledge and familiarisation of the local Aboriginal culture and community
- Able to engage and build positive networks within the local Aboriginal community
- Respect for and understanding of the client’s right to informed choice, independence and determination of services required and delivered
- The ability to work collaboratively as a member of a multidisciplinary team
