**About Monash Health**

**The Role**

We are looking for talented Registered Nurses to provide education and coordination of care to support the most vulnerable people in our Community with Chronic Respiratory Disease. This role is integral in the assessment and management of client's wellbeing, and operates from a person - centred care framework and aligns with the social model of health. We provide home based services in a timely manner to reduce avoidable presentations to hospital.

Being part of the Complex Care Program, you will be responsible for ensuring Monash Health works collaboratively with Refugee and Aboriginal Health and support the Better at Home initiative.

**About You**
- You are a dynamic Registered Nurse with a genuine passion and interest in this unique opportunity, combined with proven experience and effective clinical assessment skills
- With proven ability to work both independently and collaboratively, you readily build strong relationships with stakeholders in the delivery of client centred care
- In this permanent role your flexible availability is key to being rostered across seven days per week (Monday - Sunday), with shifts spanning from 08.00am to 20.30pm, which may include regular 8 hour shifts and 10 hour shifts when program demand is high
- As you will work across various Monash Health sites, a valid driver’s licence is essential.

Ensuring you work within the scope of practice as a Registered Nurse, and escalating to Nurse Practitioners and Medical Professionals when required

Completing all mandatory and competency assessment training

On a personal level, you are enthusiastic, motivated and demonstrate excellent communication and interpersonal skills. What You need:
Essential:

- Demonstrated recent experience as a Registered Nurse
- Current AHPRA registration
- Australian working rights
- Valid Driver's licence
- Current Working with Children’s Check
- Current Police Check

Desirable:

- Previous Community Nursing experience
- Post-graduate qualifications in critical care or similar
