Pre and Post Worker - Residential Services, Wyla
Purpose of Position
The Pre and Post Worker will deliver individual and group-based counselling and support services to proactively assist and support clients with entry and exit into suitable residential treatment. This will involve working closely with the Wyla Team Leader, the Wyla
Residential Services Team and liaising with key stakeholders.
You will work closely with the residential treatment facility to enable and support clients in and out of the treatment setting in
a way that maximises person-centred care in an integrated, supportive, and professional manner.
**Responsibilities**:
Provision of Treatment and Education Services
- Provision of competent and compassionate engagement, assessment, referrals and support to clients, using evidence-based practice principles
- Effective assessment and management of client risk
- Providing recommendations for entry to Wyla based on assessment results, screening and risk assessments (e.g. suicide and withdrawal risk)
- Working with the Team Leader and Clinical Service’s Manager to maintain and grow the occupancy of the service, managing enquiries, assessments, admissions, and client demand, developing and maintaining referral pathways.
- Coordinating admission for new residents on their first day, confirming or organising transport support and timing with the Wyla Team
- Support, assess and encourage client readiness for engagement with and admission to residential treatment
- Actively engage and develop client capacity and community supports for when clients exit residential treatment - focusing on continuing care
- Provide input and participate in the development of transition plans to support effective person-centred services, particularly when supporting admission and discharge of
clients.
- Implement and educate clients in evidence-based relapse prevention strategies.
- Ensure continuity of care by effectively transitioning clients in and out of residential treatment utilising skills and expertise in multidisciplinary teams and effectively negotiating and advocating for client support needs with other health care providers Program Management
- Develop clinical practice knowledge and expertise through active learning within the team and by engaging in professional development activities
- Participate in, and contribute to, regular case conferences, presentations and team discussions in relation to clinical practice and issues
- Actively engage in, and contribute to the development of the Wyla team
- Articulate and discuss AOD and its associated factors with the team to align service delivery with evidence-based practice
Quality Assurance
- Actively ensure procedures and relevant systems are implemented, understood and adhered too
- Conduct practice in a manner consistent with professional standards, ethical principles and the code of conduct

Key Relationships
- Work collaboratively with a range of internal stakeholders.
- Develop and maintain effective external relationships with numerous stakeholders including referring agencies, allied health professionals, Psychologists, General Practitioners, LLHD, Community Correction, Solicitors, Centrelink, other local providers
(both government and non-government).
- Ability to develop sound working and complementary relationship with designated Team Leader, Manager and Clinical Lead at all times.
Key Selection Criteria
- Tertiary qualifications in a relevant human services discipline with additional AOD qualifications highly regarded
- Applied knowledge in contemporary evidence-based drug and alcohol clinical practice.
- Experience conducting client comprehensive assessments with demonstrated capacity to make sound decisions regarding outcome of these assessments that support clients
access to appropriate treatment.
- Demonstrated expertise in residential treatment service delivery with a sound knowledge of the potential risk elements specific to this type of environment.
