We are excited to invite a talented and experienced Case Manager to become a part of our wonderful team In this role, you'll have the opportunity to provide and coordinate essential services for our valued clients right in their homes. You'll work closely with our nursing and support teams, as well as clients and their families, to create, manage, and monitor personalised care tailored to each individual's needs and plans.

**Key Responsibilities**:
**Client Assessment and Planning**:

- Undertake comprehensive client assessments and develop client profiles and care plans in collaboration with clients, their representatives, and other relevant stakeholders.
- Develop and execute personalised care plans based on assessment findings.

**Care Plan Monitoring**:

- Ensure care plans are regularly monitored and reviewed to align with clients' changing needs, including conducting regular home visits.

**Service Coordination**:

- Coordinate the provision of quality person-centered care according to clients' needs, service/care plans, and available funding within budget.
- Arrange a diverse range of in-home support services, including personal care, nursing, therapy, and allied health services.

**Advocacy**:

- Act as an advocate for clients, ensuring their rights, preferences, and choices are respected and upheld.

**Collaboration**:

- Collaborate effectively with internal stakeholders to ensure clients' needs are met, including quality, compliance, and rostering assistance.
- Establish effective communication with external service providers to guarantee seamless delivery of care.

**Documentation and Reporting**:

- Maintain precise and current records of client assessments, care plans, and ongoing communications.
- Prepare reports as needed for both internal and external stakeholders.

**Budget Management**:

- Develop and monitor client budgets to maximise access to services.

**Team Support and Training**:

- Support and direct the service delivery team, providing necessary support and training.

**Community Representation**:

- Represent Total Care Nursing at external forums and meetings, promoting the organisation within the community.

**Flexibility**:

- The above list is not exhaustive, and the role may change to meet the company's overall objectives.

**Qualifications and Experience**:

- A Certificate IV or higher in Case Management, Aged Care, Community Services, Disability Services, or a related field.
- Experience in case management or a similar role, preferably in the aged care or community services sector.
- Demonstrated understanding of the Aged Care Quality Standards and regulatory frameworks.
- Exceptional communication, interpersonal, and problem-solving skills.
- High-level computer, documentation, and budgeting skills (mandatory)
- A current and valid Australian driver's license
- A reliable, comprehensively insured vehicle
