- The opportunity- If you are looking to further your career and support the community, this is the perfect role for you- We are seeking an experienced Palliative Care Nurse to join our Palliative Care team as a Case Coordinator on a fulltime, fixed term 12 month contract in Morphett Vale. Use your passion for palliative care and hands-on leadership to ensure our dedicated team provide a quality service and make a difference to the lives of our clients and their families.- Key Responsibilities- Provide clinical leadership and support a team of Registered and Enrolled Nurses in a community setting
- Coordinate resources to ensure quality person-centred care
- Oversee clinical and other care needs for community Palliative Care clients, including complex clinical issues
- Work closely with other teams to ensure effective delivery of palliative care services
- Identify areas of development within your team, and provide mentoring and education
- Use excellent communication skills to build relationships with clients, team members, and key stakeholders, including external care providers.

- Current AHPRA registration, with experience in & a passion for Palliative Care
- Demonstrated experience in leading a team and coordinating resources
- The ability to make informed decisions and problem solve
- A strong team focus with the ability to also work autonomously
- An understanding of care delivery requirements within a community setting
- A current Australian drivers’ licence
