Mt Gravatt Community Centre is a not-for-profit, registered charitable organisation located on Logan Road, Upper Mount Gravatt. Our centre serves as a hub for the community, offering a diverse range of services and activities accessible to everyone.

**Ways to Wellness Project**: The Ways to Wellness Project is an initiative funded by the Queensland Department of Treaty, Aboriginal and Torres Strait Islander Partnerships, Communities and the Arts and is also supported by the Queensland Community Alliance. It focuses on Social Prescribing—a community network approach where socially isolated and/or lonely individuals are referred to a Link Worker who supports them to engage with meaningful community groups, activities, hobbies, services and supports to enhance their sense of wellbeing and belonging.
**The Position**:The Link Worker will play a crucial role in integrating referred individuals into their local communities to improve their health and well-being through social connection and inclusion. The role also involves working closely with GPs, Primary Care Teams, Allied Health providers, and other organisations to enhance referral pathways and community cohesion and embed evidence based and Social Prescription practices.
**Key** **Responsibilities**:
**_ Client_**_ _**_Services:_**
- Provide client-centred support through strengths-based intake and assessment and case management, utilising professional expertise.
- Assist clients in achieving personal health and wellbeing goals by connecting them with community-based groups, activities, services and supports.
- Manage comprehensive client assessments and case management, ensuring all interactions are planned and structured to improve health, connection, and well-being outcomes.
- Maintain accurate and comprehensive records, including reports for various stakeholders.
**_ Community_**_ _**_and_**_ _**_Strategic_**_ _**_Engagement:_**
- Engage actively with the community, promoting Social Prescribing and fostering social inclusion and belonging.
- Develop and maintain relationships with internal and external stakeholders, including medical professionals, government and non-government agencies, and community organisations.
- Identify and implement continuous quality improvement opportunities aligned with MGCCI values.
- Participate in mandatory training and professional development programs as required.
**Selection** **Criteria**:
- Tertiary Degree in Social Work, Psychology, Human Services, Public Health/Health Promotion, or a related field.
- Minimum of two years of relevant community work experience.
- Strong understanding of evidence-based frameworks including person-centred and trauma
- informed approaches.
- Demonstrated ability to work with disadvantaged and vulnerable individuals or those facing significant barriers.
- Excellent clinical reasoning, problem-solving, and communication skills.
- Experience in developing and maintaining relationships with various stakeholders.
- Ability to work autonomously while contributing to a collaborative team environment.
- Flexibility to work occasional evenings to meet service needs.
- Valid Driver’s License, Police Check, and current Blue Card required.
**Desirable** **Skills**:
- Knowledge of local agencies and services in Brisbane’s Southside.
- Familiarity with social prescribing.
- Experience in managing databases.
- Marketing experience related to service promotion (not required but advantageous)
