Why this role matters
The Salvation Army Doorways program provides person-centred care to community members who are in a financial crisis or suffering other forms of hardship or distress. Our practitioners are mindful of trauma informed practices, working within a strength-based context with holistic principles to build individual capacity, resilience, and well-being.
The role you could play
- To provide case management support in a community setting, supporting individuals identified through the RSL Program to secure housing, experience improved social inclusion and achieve housing stability,
- This support is achieved through person-centred case management incorporating resilience, trauma informed and strengths-based practices whilst working effectively with other members of the team to provide a comprehensive service that reflects the vision, mission and values of The Salvation Army.
- This role is within the Homelessness Stream of the Social Mission Department.
You will successfully
- Establishing relationships with individuals that access the service that encourage self-determination and participation
- Providing case management support to individuals in accordance with TSA case management models and standards of practice
- Ongoing assessment of the individuals’ presenting issues, vulnerabilities and needs from initial intake to exit
- Individual case planning in collaboration with the individual including a range of interventions that address the needs or goals of the individual, as well as barriers to achieving those
- Assisting individuals to access support services and establish community supports, and where needed provide advocacy for the person with service providers
- Regularly reviewing progress and outcomes with the person of individual case plans and where needed taking action to ensure the plan continues to meet the persons needs or goals
- Maintenance of accurate, up to date records and data in accordance with TSA standards and procedures using TSA information systems
- Identifying risks to individuals, staff and TSA and addressing where appropriate of bringing to the attention of the Team Leader as soon as possible
This is a full-time position, based in Spring Hill, QLD. Salary and conditions are in accordance with the Crisis QLD Lvl 2 - Accom Srvcs Wkr A Lvl 2.
You will bring
- Tertiary qualification in Community Services, Social Work or related field. A degree level qualification is preferred.
- Relevant experience in a social service environment including in client assessment and case management
- A national police record check is required
- QLD Working with Children Check (position is located at a site that provides overnight accommodation to young people and/or provides support to families in a home setting)
- Compulsory Code of Conduct, WHS and Bullying and Harassment training modules are completed and up to date
- Driver’s Licence
- First Aid Certificate
