**About the Carer Gateway Program...**
Wellways Australia is the first point of contact for all Australian Government Carer Gateway funded services across Queensland, aiming to help Carers access practical information and advice and connect with services in their local area.

Carer supports and services will be delivered using a network of local Carer Gateway regional service providers Carers and those who provide a caring role to someone in their life who has a disability, mental health issue, chronic health condition, terminal illness or are frail-aged.

**Your new role...**
The **Palliative Care Specialist** is responsible for undertaking intake, assessment and planning services for carers accessing support provided by the Carer Gateway across Queensland.

This role will have a specific focus on Carers and/or care recipients who have a palliative diagnosis, to assist in accessing appropriate services. The objective of the role is work with carers over the phone to access information, support and referrals relevant to their caring role. This role will also include an outreach and face to face component.

Supporting Carers and/or Care Recipients with a **palliative **diagnosis with a high level of customer service, ensuring carers are listened to with patience and non-judgement. You will also assist carers in navigating and accessing palliative services. Another focus of this role will be to support Carers and/or Care Recipients who have completed an Advance Health Directive.

**Pre-employment compliance requirements**
- Qualifications and/or experience working in community services, allied health or carer/family peer positions.
- Certificate of Palliative Care, Grief and bereavement and/or other palliative care training courses.
- Evidence of working rights within Australia (birth cert/passport/visa)
- Satisfactory National Police Records Check (within the last 12 months)
- Working with Children's Check
- NDIS Worker Screening Check
- NDIS Worker Orientation Module Certificate - free online course
- COVID 19 Vaccination Certificate with Booster or valid medical exemption
