**We are looking for the “right attitude”**. Reporting to the Manager, in this role you will be responsible for managing a portfolio of around 45 clients. Your focus will be delivering a highly supportive care package. Portfolios are assigned based on LGA’s.

- Excellent customer service skills
- A proven ability to take a holistic approach to client needs and service delivery
- Exceptional interpersonal and communication skills to demonstrate empathy and understanding to older clients and their families
- An understanding of the issues experienced by aged care clients and their carers in the home and the differing, social, spiritual and cultural needs of clients
- The ability to use initiative and work under pressure
- Competency in Microsoft Word and Excel programs

**What your days will look like;**
- Provide Home Care Packaged clients with appropriate information relating to their care needs
- Undertake client assessments and care planning. Monitoring and Reviewing these regularly
- Assist clients in understanding their package spending and the choices available to them
- Practice within applicable legislation while recognising and respecting the client’s right
- Actively promote and refer clients to BlueCross services
- Implement best practice quality models of assessment, care plan development, implementation, and evaluation
