Case Manager

As a Case Manager, you will play a vital role in supporting children and young people in Out Of Home Care (OOHC) to achieve their highest potential. You will work collaboratively with carers, birth parents, and other stakeholders to create a safe and secure environment for these children to thrive.
This is an exciting opportunity to join our team of dedicated professionals who are passionate about improving outcomes for vulnerable children and young people. As a Case Manager, you will be responsible for case managing children and young people in OOHC, providing support to their carers and birth families, and ensuring that their needs are met.
You will work holistically, taking into account the individual needs and circumstances of each child or young person. Your role will involve engaging with clients, conducting comprehensive assessments, developing and implementing client plans, and facilitating transparent communication with relevant agencies.
This role requires someone who is passionate about making a positive difference in the lives of children and young people. If you have a strong understanding of child protection issues and client vulnerabilities, excellent written and verbal communication skills, and the ability to build strong working relationships with different stakeholders, then we encourage you to apply.
