About the Role

We are seeking an experienced and skilled Clinical Care Coordinator to join our residential care team at St George Care Centre in Bayswater. As a key member of our team, you will be responsible for leading a team of care staff, including Registered nurses, Enrolled nurses, and carers, while contributing to a positive workplace culture.

Key Responsibilities
  • Supervise the daily delivery of care to residents by the care team, ensuring high-quality care is provided at all times.
  • Provide resident advocacy, ensuring the needs and preferences of residents are respected and met.
  • Coordinate assessments and documentation for residents, ensuring accurate care planning and relevant supporting documentation are provided in a timely manner as per Amana Living policy and Daily Admission Guide (DAG).
  • Provide education, coaching, and support to the team, addressing identified gaps in care or performance as required.
  • Review incidents and complete investigations as required, recommending corrective actions to the Service Manager and implementing strategies to close any gaps.
  • Drive and monitor ANACC assessments and process to maximize funding.
  • Investigate and manage clinical issues and client complaints in collaboration with the Service Manager.
  • Conduct and delegate clinical audits, analyze results, and lead actions to close identified clinical gaps.
  • Ensure the rostered direct care staff are appropriately allocated to provide the highest levels of care to residents for the duration of the shift.
  • Contribute to the performance appraisal process for all staff within the area of responsibility, including conducting appraisals for RNs, ENs, and Carers with the Service Manager as required.
  • Manage care consumables responsibly and within budget.
  • Be a positive role model to all staff, encouraging members of the team to actively demonstrate and promote Amana Living's philosophy of care.
Essential Criteria
  • Valid Police Clearance, no older than 6 months (or willingness to obtain at interview stage).
  • NDIS Clearance certificate (or willingness to obtain at interview stage).
  • Evidence of flu and covid vaccinations (including booster dose, if eligible).
  • Ability to pass a pre-employment medical and reference checks.
  • Registration with AHPRA for a minimum of 5 years.
  • High level of clinical expertise and currency of practice.
  • Proven organisational and time management skills.
  • Excellent interpersonal communication skills, both verbal and written.
