Primary Health Care Manager

As a key member of the Primary Health Care leadership team, this role is responsible for advancing priorities through supporting the ongoing development and implementation of assigned Primary Health Care programs, services, and initiatives.
The successful candidate will work closely with other members to implement multiple projects and initiatives that support patient-centered, community-based primary health care programs and services.
They are accountable for day-to-day operations of chronic disease teams, focusing on Diabetes Centres and the wellness portfolio across Eastern Zone, spanning multiple sites.
This position will manage performance standards and operational requirements by facilitating the development and implementation of consistent Primary Health Care, Chronic Disease management, and organizational priorities.
About You- Bachelor's degree from a healthcare discipline or related field required.
- Minimum 3-5 years recent and relevant progressive leadership experience in a healthcare setting.
- Experience in chronic disease management and primary health care, specifically diabetes management is an asset.
We value strong interpersonal and communication skills, including the ability to work collaboratively with various professionals and community individuals. A demonstrated knowledge of primary health care, health promotion, health determinants, and dynamics of health needs in communities is essential.
