Recovery-Focused Clinical Case Manager Role

The successful applicant will work in the Community Recovery Services (CRS) team, providing high-quality recovery-focused clinical case management to improve wellbeing and enhance functioning in the community for adults experiencing complex mental illness or psychological distress.

Clients require regular service contact over a medium to longer-term episode of care, with multidisciplinary treatment needs that cannot be met elsewhere. The applicant will deliver face-to-face care and treatment, working in partnership with government and non-government service providers.

Under close supervision, the applicant will provide sound interventions, achieve positive outcomes for people, contribute to the multidisciplinary team processes, and participate in quality initiatives.

Key Responsibilities:

  • Deliver face-to-face care and treatment to clients over a sustained period
  • Work in partnership with government and non-government service providers
  • Provide sound interventions and achieve positive outcomes for people
  • Contribute to the multidisciplinary team processes

Requirements:

  • Current registration issued under the Working with Vulnerable People (Background Checking) Act 2011
  • Comply with occupational assessment, screening and vaccination policy requirements
  • Comply with credentialing and scope of clinical practice requirements for allied health professionals

Selection Criteria:

  • Experience in clinical case management and recovery-focused approaches
  • Ability to work in partnership with government and non-government service providers
  • High-quality communication and interpersonal skills
