Job Summary:

National Networks Plus is seeking a dedicated and compassionate Support Worker to join our team in North Brisbane. As a Support Worker, you will work with individuals with disabilities, youths, and the aged to provide high-quality care and support.

Key Responsibilities:

  • Build and maintain positive relationships with clients and stakeholders.
  • Develop and implement individualized care plans to meet the unique needs of each client.
  • Collaborate with multidisciplinary teams to ensure seamless care and support.
  • Respond to crisis situations and provide immediate support to clients.
  • Maintain accurate records of interactions, assessments, and program outcomes.

Requirements:

  • Cert 3 in Disability & Aged or Cert IV in Youth in a relevant field - working towards considered.
  • Recent and relevant experience in a similar role.
  • 2 professional references.
  • NDIS Worker Screening (or willing to obtain).
  • Paid Working with Children check (or willing to obtain).
  • Current First Aid and CPR (or willing to obtain).
  • Unrestricted Australian Drivers Licence (required if providing community access support, driving facility vehicles).
