Job Overview
We are seeking a highly skilled and compassionate Registered Nurse to join our team in delivering high-quality nursing care to residents. As a key member of our multidisciplinary team, you will be responsible for assessing the needs of our residents, developing tailored care plans and delivering person-centered care that promotes their physical, emotional and social well-being.
Main Responsibilities:
- Deliver comprehensive assessments of residents' health needs, identifying opportunities for improvement and developing targeted care plans that meet their individual requirements.
- Monitor residents' health, administer medications and perform necessary treatments, adhering to best clinical practices and infection control protocols.
- Supervise and support nursing and care staff to ensure optimal patient care, providing guidance and mentoring as needed.
- Collaborate with healthcare professionals, families and multidisciplinary teams to provide seamless care coordination and effective communication.
- Assess and manage residents' chronic conditions, ensuring timely medical interventions and accurate documentation.
- Educate residents, families and caregivers on health management and well-being, promoting self-care and independence.
Requirements:
- Bachelor's Degree or Diploma in Nursing (or equivalent qualification)
- Experience in palliative care, dementia care and rehabilitation nursing, with a strong understanding of aged care quality standards and regulatory compliance.
- Certification in First Aid and CPR, with the ability to work independently and as part of a multidisciplinary team.
- Excellent communication, leadership and problem-solving skills, with a focus on delivering exceptional patient outcomes.
