Relationship Manager

Hearth is a NDIS Registered Service Provider that nurtures, supports, and engages with our participants and support workers to provide the highest standard of care.

We achieve this through fostering and cultivating long-term and sustainable relationships. As a central point of contact for support workers and participants, the Relationship Manager role is core to what makes Hearth different.

  • Ensure participants, their family members, support workers, and stakeholders have a clear & consistent point of contact.
  • Create and develop relationships between participants and their team of Hearth Support Workers.
  • Assess participant needs and develop Hearth's support program.

Responsibilities:
  • Maintain relationships with support coordinators, house supervisors, and other external stakeholders.
  • Lead Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their goals.
  • Report on client progress and prepare submissions for NDIS plan reviews as required.
  • Coordinate staff rosters and resolve issues proactively.
  • Audit case notes and client reviews to reflect forward planning in line with the participant's NDIS plan.

Requirements include proven experience in the disability sector, supervisory or managerial role, administrative and business communication skills, time management and organisational skills, and strong interpersonal skills.
