Community Health Professional

As a community health professional, you will have the opportunity to work with vulnerable individuals, empowering them to achieve stability and independence.

This role focuses on capacity building rather than direct clinical care. You will conduct comprehensive health assessments and deliver clinical and psychosocial interventions.

Key Responsibilities:
  • Comprehensive Health Assessments: Conduct thorough health assessments to identify clients' needs and develop effective intervention plans.
  • Clinical and Psychosocial Interventions: Deliver evidence-based clinical and psychosocial interventions to support clients' physical and mental well-being.
  • Case Planning: Collaborate with clients and their support networks to develop and implement case plans that address their unique needs and goals.
  • Record Keeping: Maintain accurate and up-to-date records of client interactions, including assessment results, intervention plans, and progress updates.
Requirements for Success:
  • Current registration as a Registered Nurse (RN) or Enrolled Nurse (EN)
  • Minimum 3 years' experience in community-based nursing, mental health, or alcohol and drug sectors
  • Proven ability to work collaboratively with multidisciplinary teams and provide culturally sensitive care to Aboriginal and Torres Strait Islander communities
  • Experience working with trauma-informed care principles and familiarity with cultural considerations
  • Valid driver's license and Working with Children Blue Card
