As a Support Worker, you will play a vital role in assisting vulnerable and marginalized older persons through high-quality case management, social work, and outreach services. You will use evidence-based approaches to provide individualized assistance using clinical frameworks and modalities.

Your goal is to empower individuals to recognize their self-efficacy and make long-term changes by taking control of their own journey. This includes providing support and assistance with daily living skills, connections with the community, personal and social development, employment, education, and training.

The successful candidate will also assist participants with activities of daily living, money management, health, and wellbeing, as well as recreational activities. They will work collaboratively to help clients throughout their journey from crisis to recovery and long-term change.

This role requires adaptability and flexibility to work with individuals with diverse special needs and frail, older people living in the community. We value diversity, dignity, and respect for all individuals regardless of their background.
