Job Summary

We are seeking a skilled Case Manager to join our team at Tradewind Recruitment Australia. As a Case Manager, you will be responsible for delivering high-quality case coordination services to clients, ensuring their ongoing safety and wellbeing.

Key Responsibilities
  • Develop and implement individualised case plans that address client needs and promote independence.
  • Provide culturally appropriate support to clients from diverse backgrounds, including those with complex physical and mental health issues.
  • Collaborate with internal teams and external stakeholders to meet case plan objectives and ensure timely and accurate completion of incident reports, risk assessments, and safety plans.
  • Refer clients to appropriate support services and maintain up-to-date records.
  • Promote a safe and respectful environment for clients, especially children.
Requirements
  • Strong experience in case coordination within the Human Services sector.
  • Demonstrated ability to conduct needs assessments and implement care plans.
  • Proven experience working with individuals from culturally and linguistically diverse (CALD) backgrounds.
  • Excellent written and verbal communication skills, with strong analytical abilities.
  • Ability to work autonomously and in multi-disciplinary teams, managing competing priorities.
