Job OpportunityWe are seeking a motivated and values-driven Case Manager to provide case management support to residents in the Open House Program.
Key Responsibilities- Support clients to achieve their goals within established case plans, including attending appointments and advocating for clients with other service providers.
- Undertake intake and assessment with clients to develop a shared understanding of their situation and related problems and strengths.
- Maintain accurate and up-to-date client records and data in accordance with TSA standards and procedures.
- Take preventative action in relation to potential high-risk situations, as per policy and procedure.
- Advocate on behalf of clients with various government and non-government agencies, both verbally and in writing.
- Establish effective relationships with clients that encourage self-determination and participation.
- Degree qualification preferred in Social Work, Mental Health, or Welfare, or a minimum Certificate IV in Community Services with related field work experience and demonstrated high-level knowledge in the sector.
- Demonstrated experience in engaging clients who encounter the Justice System and/or Corrective Services, and willingness to assist in the provision of support in areas of homelessness, alcohol and other drug dependence, and mental health issues.
- A National Police Record check and NT Working with Children Ochre Card check are required prior to commencement.
- A current NT driver's license and proof of eligibility to work in Australia are also required.
