Job Description

We are seeking a skilled professional to join our Allied Health team as a Social Worker/Wellness Facilitator.

The successful candidate will play a key role in delivering exceptional aged care while supporting our wellness and reablement philosophy.

Key Responsibilities:

  1. Engaging and enrolling new clients referred to Specialised Support Services as allocated.
  2. Conducting comprehensive assessments, reassessments, and evaluation of client care needs.
  3. Providing clinical advice to clients, carers, and other healthcare professionals.
  4. Providing ongoing support including referral and transition to other clinical and non-clinical services internally and externally for clients receiving Specialised Support Service.
  5. Providing direct intervention to Allied Health clients in line with evidence-based practice (EBP) and current guidelines.
Required Skills & Qualifications
  • Relevant tertiary qualifications in Nursing, Social Work, or Allied Health.
  • Current registration with the relevant Australian Regulatory Agency, particularly to health issues relevant in the Community.
  • Experience in community aged care or disability with a demonstrated understanding of clinical services as applicable to older people.
  • Experience in the organisation and facilitation of group programs to the aged community.
  • High-level administration and computer skills including proficiency in the MS Word suite.
